Supported living housing provides a service by Dementia Partners Limited that helps people with a range of support needs retain their independence whilst still having that extra physical and emotional support needed to lead completely fulfilled lives in their own homes.
Your inquiry will be sent to our team usually by your social worker, to visit you.
We’ll arrange a visit in person to get to know you a little better. We’ll talk about your desire to move to supported living, support need and also your short, medium and long term plans.
We’ll arrange visits to our 2 housing schemes, in Harrow so you can have a look around with your friends, family, or social worker and ask our team any questions you might have. You would also have the opportunity to meet the other tenants.
Your social worker will be in touch with you to discuss suitability of placement – they will keep you up to date throughout the process.
Once the placement is approved by your social worker, we can set a move-in date and will be on hand to support you through the whole transition process to make the move as smooth as possible. We will support you to apply for housing benefits, other benefits, change of address, etc
